I undertand that I am entering the Team Legal, Inc. web based medical records retrieval and storage system. I understand that this site follows strict HIPAA (Health Insurance Portability and Accountability Act of 1996) regulations and standards to protect the privacy of individually identifiable information. I understand that all patient medical information is considered strictly confidential and must be kept confidential at all times. I understand that any unauthorized or improper disclosure of confidential patient information is a violation of State and Federal laws. I understand that it is the responsibilty of both Team Legal, Inc. and myself to take every possible precaution to maintain strict confidentiality of not only the patient medical information contained in this site but also the secured and protected user ID's and passwords which grant access to this site. I understand that if at any time I suspect or know that a confidentiality breach has or may occur, that I will notify Team Legal, Inc. management immediately so that they may take action to eliminate or mitigate the extent of the confidentiality breach.
